MRI, especially diffusion-weighted MRI, has been used in stroke trials to establish the presence of an acute stroke and quantify its size. Imaging plays a role in patient selection for stroke trials. Perfusion/diffusion mismatch assessment by MRI provides a marker for salvageable tissue in trials of neuro-protective compounds. This can enrich a stroke patient population with patients most likely to respond to the treatment.
